In a world that largely struggles to properly care for the old and frail, Curana Health’s mission is to improve the health, happiness, and dignity of senior living residents. We do this by providing a unique blend of industry leading onsite primary care, onsite medical clinics, as well as participation in Accountable Care Organizations (ACOs) and Medicare Advantage Special Needs Plans (under the name AllyAlign Health). Between 2021 and 2022, we successfully merged five high-quality medical groups and a health plan under one umbrella in order to drive improved outcomes for our senior living residents.

Our model is unique in that we focus on delivering a population healthcare approach to vulnerable, high-risk patients residing in senior housing settings. Our commitment to value-based financial models means that our clinicians are trained and are incentivized to provide proactive, preventative care before a patient becomes sick enough to go to a hospital – which is both costly and traumatizing for older adults.

Today, Curana Health spans 28 states and serves more than 1,000 senior living locations across all settings, including independent living, assisted living, affordable housing, skilled nursing, and long-term care.

  4. If you are providing written answers for your submission, you must provide an answer to this second question: Outline the organization's achievements since the beginning of 2021 that you wish to bring to the judges' attention (up to 250 words):

Total 252 words used. If the word count is more than 25 words over the limit, please deduct one (1) point from your score.

Between 2021 and 2022, we merged six organizations with different cultures, systems, and processes into one united company. Highlights include:

    • Introducing an industry-leading, value-based organization marrying in-home primary care, brick and mortar onsite clinics, and health plans specifically geared to the unique needs of senior living residents
    • Providing best-in-case care to over 100,000 patients per month and doubling our revenue
    • Creating a new name, logo, brand standards, and website
    • Integrating org charts from six organization and identifying leaders for clinical, health plan, and shared services
    • Collaborating on a shared mission, vision, and values
    • Moving existing patients from five medical groups to one central electronic health record system and deploying a population health technology to support point of care access to vital care delivery information
    • Building proprietary software for providers to track Relative Value Units (the number of patient encounters/visits and projected compensation)
    • Building risk management training, including medical malpractice, OSHA, proactive training, safety
    • Centralizing multiple payroll systems to one platform for time sheets, training, reviews, and benefits management
    • Merging billing, accounting, email, shared drives, and coding systems
    • Creating a competitive benefits package and putting metrics in place for workforce planning and diversity
    • Building online e-commerce stores to order scrubs, logoed apparel, giveaway items, and print materials
    • Designating new headquarters and a centralized meeting space in Austin
    • Onboarding 1,000 employees into a virtual-first company (while our 600+ clinicians provide onsite services, the rest of the organization is virtual)
    • Mailing welcome kits to every associate, including a T-shirt, mug, and other items
  5. If you are providing written answers for your submission, you must provide an answer to this third question: Explain why the achievements you have highlighted are unique or significant. If possible compare the achievements to the performance of other players in your industry and/or to the organization's past performance (up to 250 words):

    Total 216 words used.

Curana Health is addressing one of the largest looming financial crises in the U.S. – the cost of safe, affordable senior housing and healthcare services. We’re different in that we are not just addressing the need for convenient, high-quality primary and preventive care services, but that we also offer solutions for financing this type of care by leveraging existing CMS models (ACO and Medicare Advantage plans).

In addition to successfully merging six organizations in 2021-2022, our achievements are significant because we provide a one-of-a-kind blend of:

    • Onsite primary care (rounding to residents’ rooms)
    • Onsite medical clinics for preventative care, sick visits, lab work, wound care, blood pressure clinics, and more
    • Highly rated Health Plans specifically designed for senior living residents who have multiple high-risk chronic conditions with proven clinical results

Other organizations in our space do not provide all three components on a national level. Our approach is proactive and evidence-based – with a specialized team of geriatricians, physicians, advanced practice providers, as well as specialists in psychiatry, neurology, podiatry, and physical medicine and rehabilitation.

In a short amount of time, Curana Health has radically changed the way nursing home operators approach care for their long-term care populations and continue to advocate for ways to improve the health, happiness, and dignity of senior living residents.

  6. You have the option to answer this final question: Reference any attachments of supporting materials throughout this nomination and how they provide evidence of the claims you have made in this nomination (up to 250 words):
