Nomination Title: Anthem Foundation Corporate Social Responsibility Impact

Tell the story about this nominated CSR program as it has been carried out since January 1, 2014 (up to 650 words). Focus on specific accomplishments, and relate these accomplishments to past performance or industry norms.

Currently, there are over 1.5 million nonprofits in the U.S. These exceptional organizations make up our communities' hidden infrastructures - safety nets that educate us, keep us healthy, and protect us when we are most at risk. Accordingly to the 2014 State of the Nonprofit Sector Survey, many organizations and communities are being left behind. 80% of organizations reported increases in demand for services and 56% were unable to meet demands. The resources identified as most valuable to meeting community needs were financial, subject matter expertise and skilled human resources.

As a company committed to transforming health care with trusted and caring solutions, Anthem has tremendous industry experience to lend to filling health improvement gaps. We believe corporate responsibility is intrinsic to fulfilling our vision to be America’s valued health partner. Central to our CSR efforts is leveraging all our collective resources – health care expertise, industry insights, employee talent, public policy advocacy, and charitable contributions - to create a healthier generation of Americans.

By having a comprehensive, focused approach to how we support our communities, our goals are reached by:

• Dramatically lessening the risk of chronic health conditions;
• Decreasing healthcare costs;
• Strengthening the health of 37M in our health plans, 67M+ individuals served through our subsidiaries, our nearly 50,000 employees and the communities we serve.

Anthem utilizes a signature approach to attaining goals - “Healthy Generations”, a multi-generational initiative targeting specific disease states and medical conditions. Focus areas include:
• Prenatal care during first trimester;
• Low birth weight babies;
• Cardiac morbidity rates;
• Long term activities decreasing obesity and increasing physical activity;
• Diabetes prevalence in adults;
• Adult pneumococcal/influenza vaccinations;
• Smoking cessation;
• Programs benefiting behavioral health efforts and individuals with disabilities.

What makes “Healthy Generations” unique from other industry CSR programs is the use of Anthem’s own internal industry State Health Index, which incorporates public health data from states where Anthem’s licensed affiliates do business. The index allows the Foundation to pinpoint the most critical health issues faced by each community. By utilizing a real-time snapshot of the health of the community’s population, the Foundation is extremely targeted in solving health issues ranging from abnormally high rates of diabetes to the prevalence of childhood obesity within local elementary schools.

Anthem received thousands of nonprofit requests for support in 2014. To supplement financial resources, we leveraged our highly skilled workforce of nearly 50,000 - to help lead “Healthy Generations” efforts. Anthem’s employee support is channeled through Associate Community Engagement Programs: Associate Giving program providing a 50% match of associates’ campaign pledges; Dollars for Doers, enabling associates to earn their favorite nonprofits grants through volunteer service; Volunteer Time Off; and Anthem Cares Fund, enabling associates to provide financial assistance to colleagues in need due to personal hardship or natural disaster.

The “Healthy Generations” program, paired with Associate Engagement programs, has been a highly visible way for Anthem to show our CSR support. In return, we have benefited from enhanced perceptions of our company - which is particularly relevant as we face the challenges of national health care reform.

More importantly, we have helped improve the health of millions by:

• Committing $12.4M to American Heart Association to educate 100 million people/year how to perform the lifesaving Hands-Only CPR technique, anticipated to double/triple survival rates.
• Partnering with Boys and Girls Clubs of America to combat the growing rate of childhood obesity and helping nearly 8 million kids through the holistic health/wellness Triple Play program.
• Expanding CATCH Healthy Habits program, an intergenerational effort to fight childhood obesity and sedentary adult habits by partnering with elementary school children with employees/volunteers over 50, to 200+ communities.
• Partnering with American Cancer Society to reduce cancer-screening disparities and increase education for populations at greatest risk for breast, cervical, and colorectal cancer.
• Partnering with March of Dimes to teach care skills and provide support networks to women/children to reduce racial disparities in poor birth outcomes.
